Providing women and families with hands on support, information, and resources is the heart of programs that offer care coordination. Our region is fortunate to have a number of programs (in each state) that provide this service to women and children.
At the present time, there are 23 different Federal Healthy Start programs across our eight states. These sites are listed in our compendium – click here to access it. To learn more about the National Healthy Start Association click here. Currently these programs are engaged in a national interconception learning collaborative. Each site has selected one area of focus such as maternal depression, reproductive life planning or healthy weight, and is partnered with colleagues from across the country with a similar interest. This work is producing a lot of new knowledge and improved services. In our region, these community-based groups are key partners.
Each state in our region also has at least one Nurse Family Partnership. To learn more about Nurse Family Partnerships click here.
Home Visiting has been one important maternal and child health component of Health Care Reform. Federal funding for these programs is already in the process of being allocated to some states. To learn more about home visiting programs and resources click here.
For many years, a number of states in our region have also provided comprehensive case management for low-income pregnant women. A number of those programs are being cut or undergoing significant changes due to budget restrictions. Keeping an eye on the status of Maternity Care Management Programs for mothers with Medicaid for Pregnant Women is something this Initiative will also seek to do.
